| You are more then welcome to visit our office
informally to discuss your requirements. Alternatively, we
can arrange a meeting at your home. This meeting gives us the
opportunity to meet you, your family, your social worker (if
applicable) your friends and neighbours. |
| What happens next? |
| Parkhouses Independent Living
Services will prepare a detailed personalised support plan that includes a complete breakdown of care, costs
and the proposed support worker for approval. |
| Finally and most importantly. |
| You can meet the proposed support worker before
the support plan is initiated. We believe this
relationship is a very important part of the plan and that the
quality of the service we provide will be central to this
relationship. |
| Once the plan is initiated, we will continually
check on what we are doing, talk and listen to our staff,
service users and external professionals to see how our service
can be improved. |
| We call this our Service Charter and it will
involve: |
| ● Regular
supervision meetings between each support worker and their
supervisor to identify training needs. A well trained and motivated team
will ensure quality of support for service users. |
| ● Undertake
careful checks on all service user files, electronic monitoring
and other records ensuring all legislative and service charter
commitments are met. |
| ● A
minimum of six monthly
reviews for all service users by the manager unless required
earlier. |
| ● Undertake an
annual survey with service users and their relatives or
representatives, to gain feedback and opinions on the service
and how this may be improved. |
| ● To
ensure that all feedback is reviewed and where applicable
introduce new innovations (or improvements in processes and
procedures) suggested by service users and other relevant
persons. |
| ● Publish our
feedback from the annual survey and our service record including
call monitoring and response statistics and offer copies to our
service users. |
